Welcome! We are so glad you will be joining us at Sierra Foot & Ankle. We look forward to getting to know you as we come along side you in this journey for your foot and ankle health. Whether you are here for a general check-up, are dealing with chronic foot pain, have an injury, or face other concerns, we are dedicated to making your experience in our office comfortable. We understand that your time is valuable and your health is important to you, so we make it our priority whenever you come in to respect your time and approach all conditions with attentive, compassionate care.
During your first visit, we will record background information like your medical history, as well as get to know a little bit about you and your current concerns regarding your feet. This is also a time for you to get to know us better and ask the questions you may have. Our expert staff will be on hand to help you with everything you need at any point during your appointment. Keep in mind, because this is your first visit with us, the time needed to efficiently and accurately evaluate and treat you may be longer than future appointments, so plan accordingly. If you will need to change your appointment for any reason, please contact us as soon as possible.
We do participate with many different HMOs, PPOs, and other major health care providers, but we do not accept all of them. Additionally, not all plans cover all services. If you are not sure if your insurance is accepted, call ahead to verify it. You will be expected to handle all costs not covered by your plan.
We do have certain information we are required to have before you can be seen. We understand that filling out patient forms does take a while, and can add to the time you spend waiting in our office before your appointment. That is why we have made them available for you to download and print or to submit online. There multiple pages of varying lengths to fill out, so set aside 15-20 minutes to complete them all. Please fill out the information as completely as you can—all the required areas must be filled out to be able to submit them through the website.
